Return to Home™ Program
Sherrill House’s Return to Home™ program is an aggressive rehabilitation program designed to maximize each patient’s potential in the shortest period possible, so that he or she can return home. While each patient is unique, the Return to Home™ program begins with a comprehensive, interdisciplinary care plan in which patients and their families are urged to participate. The rehabilitation team encourages the patient to reach his or her highest level of functioning just as quickly as possible, while maintaining a positive outlook.
The Return to Home™ program is particularly appropriate for those who require rehabilitation following orthopedic surgery or other illnesses. For example:
Orthopedic- Hip replacement
- Knee replacement
- Multiple fractures or trauma
- Arthritis
- Amputations
Neurological
- Stroke (transient ischemic attack – TIA or cerebral vascular aneurism – CVA)
- Brain injury (traumatic brain injury - TBI or acquired brain injury - ABI)
- Peripheral nerve damage (neuropathy)
- Communication disorders
A critical part of the Return to Home™ program is the coordination of transitions both into Sherrill House, and then back to home. This involves a careful assessment of physical ability, cognitive functioning, pharmaceutical therapy, as well as an evaluation of the individual’s home circumstances to be certain that the patient, once home, is able to continue to thrive. Family members are carefully coached when leaving Sherrill House to help them with the safe and manageable transition of the resident or patient back to his or her home. The professional social workers at Sherrill House play a vital role in this transition and remain an important link after discharge.
